Cost $10 (benefits the middle school)

We MUST have a physical on file for your son before he can begin practicing in pads next week.  Please make arrangements to either attend the HCMS physicals or see his physician.

The physical form can be found under FORMS on the left menu or at the following link.  Please fill out the history on page one and bring page two with you to your son's examination.
